All patients are eligible to participate in the plan. No pre-existing conditions will prevent membership. To become a member, existing patient account(s) must have a $0.00 balance. Membership becomes active upon receipt of the contract application and payment. Membership will extend one year from the date of initiation and will automatically renew each year. A credit/debit card is required to be on file for auto-renewal.
