Dental Wellness Plan
At Cali Kids Dental, we believe that everyone deserves quality dental care and an incredible experience at every office visit. Insurance limitations and financial difficulties can sometimes keep families from getting the care they need -- that's why we developed our Dental Wellness Plan.
Eligibility
All patients are eligible to participate in the plan. No pre-existing conditions will prevent membership. To become a member, existing patient account(s) must have a $0.00 balance. Membership becomes active upon receipt of the contract application and payment. Membership will extend one year from the date of initiation and will automatically renew each year. A credit/debit card is required to be on file for auto-renewal.

Yearly Membership Options and Fees

Individual Adult (ages 18 and older) $299/year

Individual Child (ages 17 and younger) $265/Year

2nd and 3rd Membership (on same account) 15% off

4th and Greater Membership (on same account) 20% off

Family discounts are applicable for individuals in the same family account, after the first membership of equal or greater value is purchased at full price.

Plan Details and Benefits

$5 per visit for:

  • 1 simple cleaning or periodontal maintenance visit per 6 months
  • 1 Fluoride treatment per 6 months
  • Consultation for restorative and esthetic treatment
  • Oral Cancer Screening
  • Evaluations of head and neck
  • Oral hygiene instructions
  • X-rays and photographs

Plus 20% Savings on all Additional Dental Services

Services include:

  • Periodontal Treatment

Exclusions

  • All services provided at other offices
